Burger King #2731/Westtown, 1502 West Chester Pike, West Chester, October 5. Fail. Food employees observed in food prep and service area, not wearing proper hair restraints. Egg mixture observed on counter at room temperature and iced coffee mixture (containing cream) observed improperly iced and at temperature of 57°F. Both were discarded on-site. Time in lieu of temperature being used as a control for potentially hazardous foods, but tomatoes, cheese and lettuce observed time marked for 2 p.m. which is >4 hours from being placed on make line. This was corrected on-site. Maintain. A thorough cleaning of the facility must be done. This includes but is not limited to: Filters and canopy of the charbroiler; Shelves of dry storage area; Mop sink Surfaces and equipment in clean condition. A thorough cleaning of the floors, walls and ceiling in the kitchen must be done. Maintain in clean condition. Repair the following: Fill in the holes in the wall above the 3-compartment sink; Replace the plenums in the kitchen area that have been removed; Repair the trim at the wall between the mop sink and the 3-compartment sink; Organize the storage room where the soda bag in box is located; Remove the unused chicken breading station in the dumpster area.

If this is the amount of food that must be stored, an additional freezer must be installed within 2 weeks. Dishwasher thermometer was not being run thru the dishwasher. It was being used to test water temp when machine wasn't running. Battery died in thermometer during inspection. Inspector's thermometer only reached 154°F. Custodian checked breaker. It was off. Unit heated to allow rinse to reach 190°F and dish surface 161°F. Staff were shown how to use the thermometer correctly and record the information. Supply new battery for thermometer and ensure staff are running it thru the machine each day.

Do not stock product in the walk-in freezer until properly operating. Clean grease off of the walls and ceiling in the kitchen. Observed 1 live cockroach in cove base below rear prep table and 1 juvenile behind dishwasher. Use bi-weekly pest control services. Keep facility and organized. Floor coving looks loose around kitchen and prep areas, ensure floor coving is tightly sealed to avoid pest harborage.

Fusion Restaurant, 1103 West Chester Pike, West Chester, October 5. Fail. Remove all cardboard liners from shelves, tables. Do not re-use cardboard boxes for storage needs. Remove all mesh foam liners from refrigerator shelves and counters. Remove paper liners on wall by beverage station. Only durable, non-absorbent, easily cleanable liners may be used (such as plastic bins, metal sheet trays, etc). Remove residential "oster" food processor. Clean the faucet handles to all sinks. Leaks were observed from the plumbing in the following areas: under hand wash sink in sushi station; under all bays of 3-bay sink; from condensate drainline of walk-in. Make repairs to eliminate all leaks and keep plumbing in good repair. Facility no longer used the fountain soda machine but uses the connecting ice bin. Remove the old soda hoses (and mount) and soda dispensers. If these items cannot be removed, entire unit must be removed and replaced with a new commercial ice bin. Facility needs to remove all excess/unused items and clutter throughout the kitchen. This includes personal items, tools, buckets, equipment/utensils, hoses, fans, plastic bags, etc. Organize remaining items.

Staff was shown how to use thermometer but ensure all staff know how to use it and are actually using it to check the machine daily and logging the thermometer temp as well as the dishwasher gauge temps.

Note: In future, additional hand sink & 3-bay sink may need to be installed in this area. In event dishwasher doesn't work, sprayer sink along with the 2-bay sink must be set up for manual warewashing. Outdoor bar does not have a hand wash sink. As mixed beverages are prepared in this bar, a hand wash sink (with cold & cold water, soap, paper towels, signage) must be installed OR menu must be limited to canned/bottled drinks only. In main bar, no hot or cold water available at hand wash sink. Make repairs to restore hot and cold running water at this sink so staff can wash hands. In side bar, no hot water available at hand wash sink. Make repairs to restore hot running water at this sink so staff can wash hands. Front door observed propped open. Exterior doors and windows may not be propped open as this risks vector entry. Close door. At kitchen hand wash sink, no hand soap was available. Obtain and maintain soap at all times. Note: correct on-site. Clean the steel wall below the hood. Clean the floors & floor drains in kitchen, all walk-ins, basement, server station and all bars. Mops are not being hung to air dry. Install a utility hook above the mop sink in the basement. Residential pesticide chemicals and traps for flying insects (such as drain flies) observed in bars and basement. Remove such items and only have a Certified Pest Control Operator apply treatments. Post Chester County Certified Food Manager Certificate in public view. Clean the following areas: Floor below deli slicer and slicer cart in deli; Floors, walls and ceiling in rotisserie chicken section; In and around hand-sink; Around grease trap; The floor drains; Dust fans on ceiling in butcher area; Rubber floor mats; Floor below sinks and in walk-in; The floor drains; Declutter and clean storage area behind ovens in bakery; Clean the fan in cake freezer outside bakery; Remove ice buildup on ceiling in main freezer. Dairy cooler: Clean the ceilings. The handwash sink located in the women's customer bathroom area does not have water at a temperature of at least 85°F. Adjust the hot water temperature to ensure that it is at 85°F or above at all times. The handwash sink in the deli, bakery, and butcher areas were blocked and not accessible at all times for employee use. Employee moved items. Ensure all handwash sinks remain easily accessible and are used solely for handwashing purposes. Grease trap in the chicken area and the butcher area was observed not properly sealed or cleaned. The facility must have a professional company service both grease traps to be cleaned and sealed. Keep invoices for CCHD. Repair the worn-down floor throughout the bakery. Ensure the flooring is smooth, non-absorbent, and easily cleanable. Repair or replace the following: In the walk-in butcher cooler, replace the plastic stripping flaps leading to the shipping area that have mold growth. Fix the hole in the ceiling above the dishwasher in the bakery. Seal the handsink in the bakery to the wall. Repair any broken cove bases in the butcher area. Lights were observed to be out in the ventilation hood and ceiling of the rotisserie chicken area of the facility. Replace the bulbs to ensure adequate lighting. Food facility operator failed to post an original, valid Chester County Health Department Certified Food Manager in a location conspicuous to the consumer. Post a current CFM Certificate in a public location.

Capriotti's Sandwich Shop/East Caln, 1086 E. Lancaster Ave. Downingtown, October 3. Fail. squeeze bottles which are food ingredient storage containers, in the kitchen area, is not labeled with the common name of the food. Ensure to label all squeeze bottles with the common name of the food item. Turkey was held in a big container in a bain marie. The temperature was 45-47°F. The facility had no logs to verify the time the cooling process started. The facility must keep logs to verify the cooling process and times. food must be cooled from 135°F to 70°F in 2 hours and/or from 135°F to 41°F within 6 hours, after preparation. Start logs and keep them immediately for CCHD to verify during inspections. Observed food placed hot in the refrigerator in containers greater than 4 inches deep, which is not a proper cooling method. Ensure that food items are placed in shallow pans while in the cooling process. Meatballs was held at 120°F, in the hot holding unit area, rather than 135°F or above as required. The meatballs were discarded on site. Ensure the meatballs are reheated prior to placing them in the hot holding unit. The meatballs must be kept at 135°F or above at all times. Deli meats was held at 45-46 °F, in the cold holding unit area, rather than 41°F or below as required. The deli meats were discarded on site. Ensue the deli meats are kept at 41 degrees or below as required. The quaternary ammonium concentration in the sanitizing solution of the 3-bay warewash sink was 0 ppm, rather than 200-400 ppm as stated on the manufacturers use directions. The facility must call ecolab and have the dispenser fixed. The facility had an approved chlorine concentration onsite and was provided with a sample pouch of chlorine test strips. The facility must either remove the quat dispenser and use chlorine with chlorine test strips or the facility must fix the quat automatic dispenser and ensure that it has the proper concentration. The deli slicer must be washed, rinsed, and sanitized every four hours or after each use. If facility is going to clean the deli slicer every four hours then a log must be kept for CCHD staff to review during inspections. The facility needs to do a general deep clean of all of the floors in the facility, specifically underneath equipment. -Clean all sinks. -Shelves. - The vents in the storage area. Fix the hot water pressure in the men's restroom. Ensure the tubes underneath the soda machine are lifted so that there is a proper air gap from the floor drain and the tube. Fix within 48 hours. Trash receptacle / waste handling unit outside does not have a tight-fitting lid or cover. The dumpster lids must be closed when not in use. Close all of the lids. The shared dumpster area has trash on the ground and needs to be cleaned. EHS will reach out to the landlord about this. The facility needs to ensure the dumpster area is clean at all times. Women's toilet room is not provided with a covered waste receptacle for sanitary napkins. Obtain a waste receptacle with a lid for sanitary napkins. This was corrected on site during today's inspection. The floor needs to be resealed in areas of the kitchen area, specifically under the warewashing sink. Seal the floors so that the floors are smooth, non-absorbent, and easily cleanable. Soap was not available in the women's restroom. Supply soap in the women's restroom immediately. This was corrected on site. Replace the lights that are not working in the hood system. fruit flies observed in the ware washing area. Facility must take measures to control, reduce and eliminate vectors, specifically fruit flies. Complete/continue the following: Certified pest control operator must continue service Biweekly. Submit copies of reports to Department promptly after each service. Maintain facility surfaces, plumbing and equipment in clean condition and good repair. Inspect incoming shipments for vectors. Keep all foods covered in storage. Mops are not being hung to air dry. Hang the mops to air dry and clean the mop sink area. Observed chemicals being stored in back area on shelving above food, utensils, equipment. Move all the chemicals to a separate storage area away from food, utensils, and equipment.

Charles F. Patton MS, 760 Unionville Rd. Kennett Square, October 3. Pass. Five gallon jars of sauce observed in the dry storage room unlabeled. Label information was then provided. Keep all products properly labeled. Lettuce, tomatoes and pickles observed on display uncovered. These were placed under sneezeguard. Maintain. Stem-type thermometers are not calibrated. New thermometers on-site. Ensure all thermometers are calibrated and checked routinely. The bain marie next to the cooking line is not in use. Remove or use.

Conestoga HS Teamer Field (Tempo), 200 Irish Rd. Berwyn, October 3. Pass. The following equipment not observed operable and no longer in good repair. Up right freezers (2) located in home concession and visitor concession no longer working. Staff indicated the units are down and need new compressors. Repair or replace and then remove from facility. All new equipment purchased shall be commercial grade, residential equipment is prohibited.

Las Toluquenas Express Inc, 520 S. Union St. Kennett Square, October 3. Pass. Raw shell eggs stored over ready to eat foods (vegetables). Eggs must be stored on the bottom shelf. Eggs were relocated during inspection. Salsas were over 7 days old. Foods must be used within 7 days of the date you make it. Salsas were discarded.

Nori Poke Bowl, 117 E. Swedesford Rd. Exton, October 3. Fail. Observed two opened metal cans being stored in standing one door cooler in the kitchen. Metal canned foods, once they are initially opened, must have the remaining contents that were not used placed into food grade storage containers. The original packaging must be discarded/recycled, as it is subject to oxidation/rusting once opened. Provide food-grade containers today. Foods in the cook line prep top and freezers stored open with no covering. Foods were covered during the inspection. Observed in-use knife stored between table edges or between tables, an area not easily cleanable & sanitized. Two food utensils containers in the front observed stored in a container of water which is not maintained at 135°F. Maintain at 135°F or implement documentation process for using time as a control. Observed food stored directly on the floor in the walk-in cooler, rather than 6 inches off of the floor as required. Move all foods to be 6 inches off of the floor as required. Time/Temperature Controlled for Safety (TCS) ready-to-eat food prepared in the food facility and held for more than 24 hours, located in the walk-in cooler, is not being date marked. Re-train staff. Time in lieu of temperature being used in the food facility to control ready to eat Time/Temperature Controlled for Safety (TCS) foods (rice) without written procedures or documentation to verify disposition of food. Implement and maintain time logs immediately. Facility was approved for pH monitoring on rice however logs were not on-site and PIC confirmed time as a control is being used. Foods are served raw or undercooked to the customer's request; however, a written consumer advisory on the printed menus and disclosures for which foods the reminder statement applies to is not provided to the consumer. Add discloser and reminders to the printed menus. Walk-in cooler and front display cooler were measured at 42°F nearing the temperature danger zone. Front display cooler was adjusted during the inspection and measured at 41°F. Adjust walk-in cooler to reach 41°F or below. Upper interior of the large ice machine, a food contact surface, was observed to have food residue and was not clean to sight and touch. Clean and sanitize today. Old dishes and utensils observed in the handwash sink in the kitchen, indicating uses other than handwashing. Dishes were removed during the inspection. Handwashing sinks are for handwashing only. Retrain staff. Food facility was using bleach approved only for use in a low temperature dish machine. Approved sanitizer was supplied during the inspection.

North Brandywine MS, 256 Reeceville Rd. Coatesville, October 3. Pass. Observed unlabeled containers of melted butter and grilled cheese in the walk-in cooler. All TCS foods shall be clearly labeled with a name and date prepared. Storage life in refrigeration shall not exceed 7 days. Provide proper labeling. The high temperature dishwasher is broken and not in use. Equipment must be maintained in good repair. Have the unit repaired, replaced or removed. Provide a new QAC test kit at the three compartment sink. Existing test kit is worn, color comparison chart faded. Powers beverage cooler has torn gaskets. Staff have used duct tape to try and cover over parts of the gasket. Have the torn gaskets replaced. Maintain equipment in good repair. Walk-in refrigerator. Observed visible mold growth on the FRP wall paneling. Have the surfaces cleaned and disinfected. Maintain a clean and sanitary state. The food preparation sink lacks an air gap in the wasteline. Food preparation sinks are required to have an air gap in the wasteline of at least 1 inch. Install an air gap in the wasteline. Hood ventilation system was not on at the time of inspection/while convection ovens were on. Staff did not know how to power on the hood system. The hood ventilation system shall be kept in good repair and maintained on during operational hours to remove high temperatures, grease and moisture. Ensure the system is in good repair and maintained on during hours of operation.

Pottstown Sunoco, 1453 S. Hanover St. Pottstown, October 3. Fail. The Person in Charge did not demonstrate adequate knowledge of the PA Food Code as evidenced by incorrect responses to food safety questions. All food employees must be trained on the basics of food safety and food handling, especially date marking, expiration dates, cold holding temperatures, the different types of sinks and how they need to be used (handwashing sink, food prep sink, 3-compartment warewashing sink, utility/mop sink), and how to use the 3-compartment sink for warewashing. In the under-counter cooler in the kitchen area, TCS foods requiring refrigeration (eggs and peeled garlic) were not date marked. Per employee on location, this food is for personal employee use only. The refrigerator must be have a sign placed on it saying it is for employee use only, or employee foods must be kept on a designated shelf marked for employee use only. At the 3-compartment sink, the quaternary ammonia (quat) sanitizer concentration was too low to be detected, rather than between 200-400 ppm as required. The quat sanitizer dispenser must be repaired, or staff must be trained in how to manually mix sanitizing solutions. Sanitizing solutions must be tested daily with the available test strips to ensure they are between 200-400 ppm. At the 3-compartment sink, hot water was only reaching 108°F, rather than 110°F or higher as required. Repair plumbing system to provide hot water at 110°F or higher at the 3-compartment sink at all times. The inside of the walk-in cooler had spills, grime, and food debris building up on the ground, and multiple containers of beverages placed on the ground. Place all beverages and beverage containers on shelves at least 6 inches off the ground, and thoroughly clean the inside of the walk in. Food facility hot water heater is not producing enough hot water to supply all sinks at the time of this inspection. If hot water is turned on, the shower immediately turns on as well and cannot be shut off. Repair plumbing to fix shower, and to provide hot water to all sinks at all times. Hand sinks must reach at least 85°F, and the 3-compartment sink must reach at least 110°F. All handwash sinks do not have water at a temperature of at least 85°F. Repair plumbing to provide hot water at all sinks. Customer restroom must be marked as out of order until sink can produce hot water. Paper towel dispenser empty at the handwash sink in the kitchen. Hand sinks must be supplied with soap and paper towels at all times. Mops were not being hung to air dry. Mop was hung to air dry during the inspection. The food facility does not employ a certified employee as required. Within a week, provide the inspector with proof of enrollment in an approved Food Safety Manager class for a supervisory employee working at this location. Within 60 days, the employee who is taking the Food Safety Manager class must obtain full Chester County Certified Food Manager certification. Tasty Table Catering, 10 Leopard Rd. Berwyn, Follow-up, October 3. Pass. No violations.

Two Stones Pub, 843 E. Baltimore Pike, Kennett Square, October 3. Fail. Employee observed handling soiled dishes then unloading clean dishes from the dishwasher without a handwash between. Open drinks observed along the cook line. Keep all drinks lidded and stored in areas away from food and food contact surfaces, and equipment. Label spray bottle of oil on the cook line. Maintain all containers labeled with contents. At the middle bain marie on the cook line, blue cheese dressing and a sauce observed at temperatures of 48°F. These were discarded on-site. Staff indicated Caesar dressing is made with raw eggs, however, it is not clearly disclosed on the menu with an asterisk. Place on menu. The concentration of quat sanitizer in the buckets along the line was found to be 100ppm. It was found that the dispenser was not operating properly. This was corrected on-site. Maintain. Use test strips routinely to monitor. Deli slicer was observed with food debris on it. Thoroughly clean and ensure it is clean to sight and touch. Dumpster lids and side doors observed open. Keep closed at all times. A fly strip observed hanging in the middle of the storage area of the kitchen. This was removed on-site. Ensure all methods of pest control are done by a certified pest control operator and do not risk contamination of foods or food-contact surfaces. Clean the following: aFloor behind and under the cooking equipment; Floor under shelves and in cabinet in wait station. Employee personal belongings observed in the kitchen area. These were relocated on-site. Remove all unused equipment in the storage area. Bottles of medicine observed on shelf in wait station area. These were removed on-site.

Vera Pasta, Inc., 319 Westtown Rd. Suite K, West Chester, October 3. Pass. Remove all damaged, peeling caulk from hood canopy and re-caulk. Ensure oven and range are both fully positioned below the hood for the purpose of proper ventilation. Clean and sanitize the can opener. Note: corrected on-site. The hood must be professionally cleaned. Post dated cleaning sticker on hood canopy for documentation. There is no Chester County Certified Food Manager enlisted. Using approved, valid food safety certificate, complete county CFM application process and post county CFM certificate in public view.

Westlakes Cafe, 1055 Westlakes Dr. Berwyn, Follow-up, October 3. Pass. No violations.

Chartwells @ MD Lang ES, 409 Center St. Kennett Square, October 2. Pass. A thermometer for testing the dishwasher temp is provided but it was not being used to check and log daily temperatures. Staff was shown how to use thermometer but ensure all staff know how to use it and are actually using it to check the machine daily and logging the thermometer temp as well as the dishwasher gauge temps. Clogged line at the dishwasher spray sink is being worked on. Repair sink to drain properly. Complete work during next break if work can't be done during school hours.

Damas Mediterranean Grill, 260 N. Pottstown Pike Suite 50, Exton, Follow-up, October 2. Fail. Food Employee observed donning single use gloves without a proper hand wash prior. Hands were washed in the 3 compartment sink without soap. Employee washed hands in the handwashing sink and put on a new pair of gloves. Retrain staff. Raw beef stored above ready to eat foods in the walk-in cooler. Re-train staff. Ensure all raw animal products are stored below ready to eat food items. Food ingredient storage containers (squeeze bottles), along the cook line and inside prep tops, are not labeled with the common name of the food. Label all food ingredient storage containers. Sauces stored in the top of the small middle prep top at the cook line were measured at 45°F. The amibient air temperature in the bottom of the unit and foods store inside the bottom of the unit was 41°F. All food products were moved from the unit during time of inspection. THE UNIT IS NOT APPROVED FOR COLD STORAGE PURPOSES UNTIL THE UNIT CAN HOLD PROPER TEMPERATURES. Repair unit within 24 HOURS and ensure that all cold holding units are 41°F and below at all times. Cut tomatoes were held at 51°F, in the right-side prep top at the cook line, rather than 41°F or below as required. PIC stated that tomatoes had come from her house. Tomatoes were discarded during the inspection. Foods cannot be stored or prepared outside of the facility. Meat slicer, a food contact surface, was observed to have food residue and was not clean to sight and touch. Clean and sanitize meat slicer blade and exterior TODAY. Paper towel dispenser empty at the handwash sink in the kitchen. Paper towels were supplied during the inspection. This is the only handwashing sink in the kitchen and it must be supplied with soap and paper towels at all times. Employee personal medication was observed on a prep table stored next to prepared foods, and not stored in dressing rooms or identified storage area as required. Ensure all medication/first aid supplies have designated storage areas away from food.

Michelangelo's Italian Restaurant LLC, 1223 Horseshoe Pike, Downingtown, October 2. Fail. Staff did not display proper knowledge on safe hot and cold holding temperatures, sanitizer levels, cool down practices, etc. The Certified Food Manager must continue to train and monitor staff to ensure proper compliance with the Food Code is maintained. It was recommended ownership send the head chef to Certified Food Manager Classing. Conduct additional training. Small plastic dry storage shelf. Unit lower shelving is not elevated the required 6 inches off the floor and unit is not on castors. Remove or replace. New shelving or dunnage racks shall be at easily cleanable and at least 6 inches off the floor for easy cleaning. Sauce in the steam well was measured to be 125°F. Hot TCS foods shall be maintained at 135°F or greater in hot holding. Sauce reheated to 165°F on site. Monitor hot holding temperature daily. The following TCS foods were stored at room temperatures: Pizza Sauce 44°F; Mozz cheese 54°F; Garlic in Oil 55°F. Product must be maintained in refrigeration and may not be left at unsafe temperatures. As discussed with ownership, a new bain maire with upper compartments is highly recommenced. Note: Ice bath provided for pizza sauce at the time of inspection. Time tracking for pizza sold by the slice was not available. Pizza discarded on site. When using time as a control, you must properly time track the items. Provide time tracking for pizza left out at room temperature. Use or discard within 4 hours. Observed improper sanitizer levels: The measured level of chlorine in the sani-pale was over 200 ppm. Maintain chlorine sanitizer levels between 50-100 ppm. Upon interview, staff were not reaching high enough QAC levels in manual warewashing. Maintain the QAC level between 200-400 ppm. Conduct additional training and monitoring to ensure proper sanitizer levels are being maintained. Thoroughly clean the following: Small cookline bain maire; dirty interior; Walk-in cooler flooring below the condensate drain line; soiled. Clean all within 24 hours. Repair the following: Front handwashing sink; broken spigot head; Three compartment sink right spigot head; turned off at the valve due to a continuously leaking spigot. Maintain all plumbing works in good repair.

Walgreens Pharmacy #12034/E. Bradford, 300 N. Bradford Ave. West Chester, October 2. Fail. Accumulation of trash and debris observed on the ground surrounding the cardboard and trash dumpsters. Clean and maintain in clean condition. Women's toilet room is not provided with a covered waste receptacle for sanitary napkins. Clean the following and maintain in clean condition: Floor of the back storage room; Wall by handwashing sink in men's toilet room; Urinal in men's toilet room.

Wawa #8068/West Goshen, 1010 West Chester Pike, West Chester, October 2. Fail. Temperature measuring device for ensuring proper temperature of equipment is not available or readily accessible in the cold wells of the smoothie station and self-service coffee station. Provide thermometers. Due to CCHD's ventilation concerns regarding the Lang Platinum double oven, West Goshen Township evaluated this equipment and its operation and determined a hood needs to be installed. Plans have been submitted and are currently under review. Once plans are approved, complete hood installation in the timeframe to be set forth by CCHD & West Goshen Township. Note: An extension to correct has been granted. Clean the following areas: Icee unit sides, front and below; Lid dispensers for beverages (coffee area and fountain beverage area).
